Your First Visit
Please arrive 10 minutes prior to your scheduled appointment time and bring a photo ID, your insurance card, and a method of payment for your co-pay. We accept cash, checks, and credit cards. Check-in with the front desk staff and fill out any paperwork that needs updating. Alternatively, prior to your appointment, you can print new patient paperwork and bring the completed forms to your appointment.
All minors must be accompanied by a legal parent or guardian for their first appointment. For any following appointments, consent may be given by the legal parent or guardian to authorize another individual to accompany the minor patient or for the minor patient to be unaccompanied for treatment and medical care. Please read the Minor Consent for Medical Care form.
Our Edina clinic is located in the Edina East Professional Building just north of the Southdale Mall. Our clinic is located in Suite 120 and Suite 200. Check the plaques in the lobby of the building to see where your provider is practicing. There is free parking in front of and behind the building. The building is handicap accessible and has an elevator.
Our Eden Prairie clinic is located in the Flagship Corporate Center near the Eden Prairie Southwest Metro Station. Our clinic is on the third floor in Suite 370. There is a free parking lot in front of the building. The building is handicap accessible and has an elevator.
Most dermatology consult appointments are between 10-20 minutes. You can expect to spend about 30-45 minutes at the office. The duration of treatment appointments vary. Please speak with your dermatologist to plan accordingly.
After your appointment with your dermatologist, you may proceed to check out to purchase any skincare products or to make your next appointment.
Appointment Cancellation Policy
Your appointment is reserved especially for you. Should you need to cancel or change the date of your appointment, we would require a 24-hour notice.
A patient who fails to show up for a scheduled appointment without prior notice will be considered a “no show”.
A patient who no shows for an appointment or fails to give adequate 24-hour notice for an appointment may be assessed a $50.00 fee. Multiple “no show” visits may be subject to dismissal from the practice.
As a courtesy, we will do our best to send a reminder via telephone call for appointments. If you do not receive your reminder call or message, the cancellation policy will still remain in effect.
If you are running late for your appointment, please give us a call at 952-920-3808. Our physicians will try to accommodate you into their schedule if they can, however, this is not always possible and you may need to reschedule. You may also be subject to the $50.00 no show/late cancellation fee.
